I do not know if this still works ... it has been in storage for at least a couple of decades.  I'm not even sure (I doubt, in fact) it would work on the modern American power grid (standard outlet).  I had a replacement fuse, but that was shattered at some time in the past and I've discarded the debris.  The instruction insert is in Japanese and I've included that in the images below.  This was used, most likely, in a research biomechanics research laboratory in Japan.
